O Tratamento da síndrome do ovário policístico é apenas sintomático. Então baseia-se na modificação do seu estilo de vida, na possibilidade de tomar certos medicamentos, no monitoramento de possíveis complicações e no controle da infertilidade.

- corrigir os sintomas relacionados ao hiperandrogenismo ( acne , hirsutismo , etc.);
- restaurar os ciclos menstruais para aumentar as chances de fertilidade e proteger a paciente do risco de câncer endometrial;
- melhorar as anormalidades metabólicas e, em particular, os níveis de açúcar no sangue ;
- corrigir o excesso de peso , se necessário;
- induzir um ovulação se a gravidez é desejada.
Assim, este tratamento baseia-se na modificação do seu estilo de vida, tratamento medicamentoso, se necessário e monitorização de alguns sintomas. Para mulheres inférteis, soluções também são oferecidas.
ADAPTAÇÃO DE SEU ESTILO DE VIDA PARA REDUZIR OS SINTOMAS DA SOP
Tratamento da síndrome do ovário policístico
Para mulheres que sofre de SOP e especialmente estão acima do peso por exemplo , uma melhoria no estilo de vida é essencial para promover o bem-estar. Uma perda de cerca de 10% do peso inicial é então recomendada graças a:
- uma alimentação adaptada e balanceada ;
- aumento da atividade física .
A adoção de um novo comportamento alimentar e físico:
- ajuda a reduzir o hiperandrogenismo e seus sintomas. Portanto conduz em particular à redução da queda de cabelo e à redução da hiperpilosidade e da acne ;
- tem efeitos positivos na amenorréia, ou seja com benefícios potenciais na fertilidade e no humor.
A longo prazo, dessa forma, perder peso tem um impacto positivo no risco de complicações metabólicas associadas à SOP e, em particular, na redução do risco de desenvolver diabetes tipo 2.

MEDICAMENTOS PARA REDUZIR OS SINTOMAS DE HIPERANDROGENISMO NA SOP
No caso de hirsutismo, uma pílula de estrogênio-progestogênio é recomendada como tratamento de primeira linha. Este tipo de contracepção tem vários efeitos positivos:
- uma diminuição nos níveis de andrógenos;
- regulação dos ciclos menstruais;
- redução da acne , queda e excesso de cabelo .
Se a pílula combinada não tiver efeito ou não for apoiada pela mulher com SOP, o tratamento é baseado em um antiandrogênio (acetato de ciproterona) combinado com umestrogênio natural. Este tipo de medicamento é eficaz:
- em 3 meses com acne ;
- em 6 meses em hirsutismo .
No entanto, os tratamentos hormonais combinando acetato de ciproterona e etinilestradiol (como a droga Diane 35 ou seus genéricos)
aumentam o risco de trombose venosa ou arterial e o risco de desenvolver meningioma, um tumor benigno das meninges cerebrais. Portanto, é necessário tomar precauções porque:
- não deve ser prescrito em combinação com o tratamento da acne porisotretinoína ;
- é contra-indicado em caso de história de meningioma.
MONITORAR E TRATAR PROBLEMAS ASSOCIADOS À SOP
Além do monitoramento configurado em caso de excesso de peso, mulheres com SOP devem ser monitoradas quanto ao risco
cardiovascular e,no entanto, em particular, quanto ao açúcar no sangue , nível de colesterol no sangue, pressão arterial, etc.
Então um aumento do açúcar no sangue, que pode levar ao diabetes tipo 2, é tratado como tratamento de primeira linha com medidas de estilo de vida e dieta e perda de peso , se necessário.
Dos medicamentos antidiabéticos orais (metformina) podem ser prescritos, se necessário.
Os pacientes devem ser informados sobre os possíveis efeitos colaterais da metformina: desconforto, dor de cabeça, etc.
O controle do excesso de peso é acima de tudo higiênico e dietético . Em casos de obesidade grave, a cirurgia bariátrica deve ser discutida com seu
médico, levando em consideração os riscos de gravidez em mulheres com excesso de peso extremo.
SÍNDROME DO OVÁRIO POLICÍSTICO: QUAL TRATAMENTO PARA A INFERTILIDADE?
O controle da infertilidade é possível.
Antes de iniciar este tipo de tratamento, é necessário certificar-se de que não existem outros fatores que causam infertilidade, como anomalia anatômica das tubas uterinas ou, nos homens, anormalidades do esperma, por exemplo, que alterariam o processo de tratamento.
Indução da ovulação ou estimulação ovariana

Consiste em fazer micro-perfurações na camada superficial de ovários a fim de alcançar ovulações normais e gravidezes espontâneas.
Esse tipo de cirurgia permite que a ovulação seja restaurada em cerca de 50% dos casos. Pode ser oferecido:
- após falha dos tratamentos com citrato de clomifeno ou em caso de resistência a este tipo de tratamento;
- como alternativa aos tratamentos de indução da ovulação com gonadotrofinas;
- como alternativa ao uso da fertilização in vitro, que potencialmente produz mais efeitos colaterais em pacientes com SOP;
- de primeira linha se a laparoscopia for indicada por outros motivos que não o problema de ovulação (problema tubário, endometriose , etc.).
